Abstract: In order to improve system safety, it is necessary to identify its control defects and take measures to eliminate or manageharmful factors. Causationanalysis based on STAMP (CAST) method was extended, and on the basis of systematic theories, violated safety assumptions in accidents were identified built onanalysis of safety control defects. Then specific safety indicators were put forward to monitor hidden dangers in system operation. Finally, with accidentsin Taiwan railway as an example, a number of safety indicators for operation of puyuma train were proposed, including product selection, operation maintenance, organization management and other aspects. The results show that this method could detect control defects in train operation, and the safety index proposed is reasonable and effective.
