Comparative study of systematic analysis on accident causes based on 24Model and accident investigation reports: taking Xiangshui explosion accident as an example

摘要:

为促进“2-4”模型(24Model)在官方事故调查工作的广泛应用,探究事故调查报告存在的不足之处,以响水“3·21”爆炸事故为例,基于24Model分析事故原因,绘制事故原因的作用路径,并与调查报告进行对比。结果表明:响水天嘉宜公司在个人层面的不安全动作、不安全物态、安全能力,以及组织层面的安全文化和安全管理体系的缺欠导致该事故;事故调查报告并未对组织层面的原因作出详细分析,没有揭示事故发生的机制,并未准确定义和划分各类责任者;24Model在对各类原因和因素完整定义的基础上,能够以系统分析方法总结事故原因并且明确事故责任者的定义和划分。

Abstract:

In order to promote wide application of 24Model in official accident investigation, and to explore shortcomings of accident investigation reports, with Xiangshui "3·21" explosion accident as an example, accident causes were analyzed based on 24Model, and their action paths were plotted. Then, comparison was made with investigation report. The results show that causes of the accident in Tianjiayi company include unsafe acts, unsafe conditions and lack of safety capability at individual level, as well as lack of safety culture and safety management system at organizational level. In regard of causes at the latter level, the investigation report does not make a detailed analysis, without revealing accident mechanism or accurately defining and dividing various types of responsible persons. However, 24Model, based on complete definition of every cause and factor, is able to summarize the causes with systematic analyses and clarify definition of responsible persons and their liability distribution.
