    Research on classification and gradation of offshore oil accidents in China

    In order to improve statistics and analysis of offshore oil accidents in China, firstly, relevant laws, regulations, standards and guidelines were analyzed to clarify definition of these accidents and specify their gradation and classification. Then, a comparative analysis was conducted on current regulations or practices regarding accident classification and gradation of the State Council, Ministry of Emergency Management, industries of different fields, three Barrel of Oil and international oil companies. Finally, scope and definition of offshore oil accidents were proposed, as well as the new gradation and classification, including their specification or applicable situation. The results show that based on the previous classification released by the Ministry of Emergency Management of People's Republic of China, accident classification can be expanded to eight types including thirty-nine sub-types in China. And on the basis of previous gradation by the State Council, accidents can be graded into six levels in China.

    Multi-objective optimization design for safety system based on isolation-inoperability competing failure mechanism

    In order to avoid single indicator-based design flaw, a multi-objective optimization design method of safety system based on isolation-inoperability competing failure mechanism was proposed. Considering that the core of this method was thresholds setting of isolation component and inoperability component, three performance indices, including PLOAS, reliability loss, mass cost, were utilized to express influence of thresholds setting on system safety, reliability, and mass respectively. On this basis, a relevant multi-objective optimization function was established, and linear weighted method was applied to converse multi-objective optimization problem to a single one. The results show that the proposed method is suitable for solving multi-objective optimization problem based on isolation-inoperability competing failure mechanism, and Pareto solution sets can be obtained not only for three objectives optimization problem, but also for two objectives optimization problem under strong restriction of safety.

    In order to do a good job of mental health education for university students under COVID-19 epidemic,DASS-21 scale was applied to measure undergraduates'mental health status. An internet-based questionnaire was taken by 6 157 undergraduates to measure their mental health status. The results show that there are some symptoms of stress, anxiety and depression in university students. University students whose family economic status is poor and whose family economic status is affected by COVID-19 epidemic have obvious symptoms of anxiety and depression. Stress, anxiety and depression symptoms are most severe among students with severe physical and mental illness.

    In order to prevent natural gas pipeline construction accidents, causes of 32 natural accidents of such were analyzed by using "2-4" model causal factor coding system.16 causal factors involved in the accident were obtained through statistical analysis of 29 factors at 4 levels, and 5 most frequent ones were analyzed in detail. The results show that for all investigated accidents, the major reasons for them are personnel's faulty operation from the category of illegal and unsafe operation, inconformity of external conditions before construction with requirements from unsafe states, insufficient mastery of equipment and material characteristics from safety knowledge, failure to realize importance of construction procedures from safety awareness, unreasonable deployment of on-site safety supervision personnel from safety management structure defect, and continuous rain from natural factors.

    In order to break limitations of relationship analysis between safety management, risk management and emergency management, and to give full play to management effectiveness of them based on perspective of Macro-security, literature review method, speculative method and comparative analysis method were used to sort out management connotation and development course of them. Internal relationship was analyzed from five aspects such as research field, theoretical paradigm, responsible subject, management stage and system development evolution. The results show that safety management, risk management and emergency management research fields overlap and respectively focus on accidents, risks and emergencies. Theoretical paradigms are blended, and management basis is interoperable among them. Responsible subjects are the same, and all emphasize participation of all members. There are differences in management stages and focus. Development process of management system is different.

    Research on modelling of safety system emergence and its key issues

    In order to promote development of basic theory of safety science, research on emergence of safety system was carried out based on system emergence thought implied in safety issues. Firstly, emergence thinking contained in safety theories was extracted through dialectical analysis, safety system emergence was defined and its conceptual model was established. Then, the concept was analyzed from multiple perspectives through dimensionality reduction analysis on complexity of security system. Thirdly, key issues and directions of safety system emergence research were proposed, followed by summary of their significance to safety system emergence research. Finally, safety system emergence was used to provide some explanation about coronavirus pandemic. The results show that system emergence is an important property of the complex safety system and it is an outcome of complex interrelation within system on the basis of its related concepts. Safety system emergence provides a brand new perspective to study safety issues and deserves deeper and comprehensive research.

    Classification of 16 hot and humid environments

    In order to explore influence of high temperature and high humidity environment on human body, such environment was simulated and heat stress experiment conducted by using environmental chamber to obtain data. With temperature for experiment at 34, 36, 38 and 40 ℃,Relative Humidity (RH) at 60%, 70%, 80% and 90%, a comprehensive physiological index F was proposed based on three physiological parameters which were oral temperature (tc), Heart Rate (HR), skin temperature (tsk) and two subjective parameters, Thermal Sensation Vote (TSV) and Fatigue Score (FS). Then, experimental working conditions were classified through cluster analysis according to Fscore for each subject, and discriminant function was built using discriminant analysis to establish segmentation threshold. The results show that 16 high temperature and high humidity environments are classified into three types whose range of F score is 28.14-39.76, 39.17-45.21, 44.13-52.39 respectively. And they can be defined as generally harsh environment, moderately harsh environment and severely harsh environment

    In order to correctly understand what are the direct causes of an accident and facilitate enterprises to establish the dual-prevention mechanism that has been promoted in China work safety management currently.An accident case was introduced and a clear definition of accident was put forward.The accident was decomposed into several events,and the direct causes of the accident were analyzed.The contents of hazard and hidden danger and the relationship between them were analyzed according to the current domestic and international regulations and standards and the theory of accident causation.The results show that the direct causes of an accident can be unsafe acts and unsafe conditions,or energy and its exchange,that an accident,accident causes and accident consequence are corresponding events respectively,that both hazard and hidden danger are causes or potential causes of an accident,and that there are three views on the relationship between hazard and hidden danger,including equivalent,inclusion and parallel,and the advantages,disadvantages and compliance are different,and among them,the view taking hazard as hidden danger is in accordance with the current regulations and standards.

    In order to solve the problem that relevant concepts of safety risk classification control are not unified and confusion used in actual work safety, the connotation and definition of six basic concepts, including hazard, accident potential, cause of the accident, accident category, hazardous and harmful factors and safety risk, were summarized and clarified based on comparative analysis of relevant domestic standards and scholars' opinions. Then the interrelationship between concepts was discussed and the conceptual relationship diagram of safety risk classification control was proposed. Finally, based on basic concepts and safety management, a full-time safety risk control model based on key time points was established which clarified relationships between safety risk management and accident potential investigation and control, accident prevention and emergency response. The accident control system of "5E" countermeasures was constructed which clarified the relationship between accident prevention, risk assessment and emergency management. The results show that basic concepts are the basis of safety risk classification management and control, and that the relationship among safety risk management and control, accident potential investigation and control, accident prevention, risk assessment and emergency response is related to the implementation effect of safety risk classification control, which should be clarified and standardized.

    In order to make 24Model achieve better application effect in tracing the cause of accident and formulating accident prevention countermeasures, the detailed scientific explanations of 24Model are critically important. By means of systematic literature review and comparative studies, the logical structure, causation modules and main features of 24Model were deeply examined. The research results show that: the primary theoretical foundations of 24Model are the series of Domino Theory proposed by HEINRICH and improved by BIRD etc., and the series of Swiss Cheese Model proposed by REASON and clarified by SHAPPELL & WIEGMANN, and another one is the Model of Managing for Outstanding Safety proposed by STEWART. The logical structure of the static form of 24Model is established by cause-effect relationship and falls into the sequential category of accident causation models. The specific meaning of the immediate, indirect, root, and radical accident causes in static 24Model can be found from the above three types of accident causation models, management system standards by ISO and the safety culture defined by the authors of this paper. The dynamic form of 24Model takes the process of behavioral conversion to establish its logical structure, and it fully expresses its systemic, dynamic, and non-linear features, and it forms a behavioral conversion system by linking organizational behaviors, namely influence behaviors, and individual behaviors, namely operative behaviors.

    Statistical analysis on dust explosion accidents in China from 2005 to 2020

    In order to further improve pertinence and effectiveness of dust explosion prevention in China, statistical researches on 67 dust explosion accidents from 2005 to 2020 were carried out, including year, region, dust type and enterprise scale, and their causes, workplaces and consequences were also studied before some preventive suggestions were put forward. The results show that metal, coal, grain and wood dust are the four main dust types that cause explosion. Number of such accidents in large and medium-sized enterprises is small, but the consequences are much more severe than those in small and micro-sized ones. And major causes of the 67 dust explosion accidents are individuals' illegal operation, imperfect ventilation and dust removal facilities, defects in safety design of equipment and facilities, and other safety accidents. In terms of sites, metal polishing workshop, dust removal system, underground mine and dust drying tower should be paid more attention to prevent explosion. Moreover, the dust types causing more than 10 deaths in explosion accidents are namely coal, metal and grain.

    In order to effectively prevent dust explosion accidents and realize the risk classification and control of dust explosion accidents, a new dust explosion risk assessment method is put forward. Firstly, taking the possibility and severity of dust explosion as the one class index, the distribution of personnel, the layout of buildings, the equipment and facilities and others as the secondary indicators and safety management measures as compensation index, the risk assessment index system of dust explosion was constructed. Secondly, the weights of each index were determined by combining the AHP and the entropy method, and the risk assessment model of dust explosion was established to evaluate and classify the risk of dust explosion. Finally, taking the dust explosion accident of an enterprise as an example, the application of the proposed risk assessment method was studied. The results show that the classification of the evaluation indexes directly affect the risk assessment results of dust explosion, that the safety compensation measures can effectively reduce the risk of dust explosion in enterprises, and that the risk grade of dust explosion in the enterprise is general by using this risk assessment method, which is consistent with the evaluation results of the expert team at national level.

    In order to improve the safety of the production process, in-depth research on accidents with human error as the main and machine failure as the second was carried out. Based on Lawrence accident model (L), behavioral safety "2-4" model (B), trace intersection theory, and accident cumulative effect theory, combined with safety ergonomics and other related knowledge, "L-B" man-machine accident model was constructed, and the actual case was analyzed. The results show that the accident model includes seven stages: work stage, hidden danger stage, trigger stage, occurrence stage, emergency stage, calm stage and recovery stage, analyzes the causes of accidents from various angles and illustrates the development characteristics of accidents from the beginning of production activities to the occurrence of accidents later, which can provide a reference for enterprises to formulate safety measures.
